CHAIM (Check by Atypical Information Manager) Guide

At our clinic, we manage treatment plans using a system.
However, there are cases where the patient's preferences and choices result in non-standard plans that are difficult to manage within the system.
To support such situations, we have introduced "CHAIM."

If you are designated as CHAIM-compatible during your consultation, please contact the CHAIM hotline immediately: (Contact person for CHAIM) (If the operating hours have ended, we will respond as soon as possible during the next available time slot).
The CHAIM representative will listen to your situation to understand and consolidate your treatment plan.
Please always communicate the details of your plan directly to the CHAIM representative.
To prevent miscommunication, we do not accept relayed messages regarding the plan.
Please note that as the number of CHAIM representatives is limited, there may be a delay in getting back to you.
We appreciate your understanding.

*Typically, there is no additional charge for CHAIM support. However, if complex assistance is required, fees may apply.

What is CHAIM?

CHAIM stands for Check by Atypical Information Manager.
In cases where it is difficult to capture the plan within the system, there may be instances of missed instructions or time taken for reconfirming treatment policies.
To avoid such situations and ensure smooth progress in your treatment, CHAIM representatives are responsible for understanding, managing, and verifying your treatment plan.

While we do manage plans through CHAIM, we kindly request the following from patients who require CHAIM support.
Your cooperation is necessary for the successful implementation of your plan.

Request for Patients Requiring CHAIM Support:

●For CHAIM-compatible plans, please contact the CHAIM hotline (Contact person for CHAIM) as soon as possible. Please communicate directly with the assigned representative.
●Inform the doctor or nurse about the CHAIM compatibility for each treatment.
●If there are any changes to your plan during the course of treatment, please inform the CHAIM representative.
●We may require your consent confirmation through documents or emails. In such cases, please cooperate accordingly.
