As specialists in reproductive medicine, we aim to support the patients of your institution who are struggling with infertility.
To facilitate the referral process and minimize any burden on your institution and the patients, we have established a referral workflow.
If you are able to refer patients to our institution, we kindly request that you follow the workflow outlined below:

Referral Workflow

1. From your institution to the patient

  1. Please complete the "Referring Institution" section and the "Patient Information" section on the "Infertility Treatment Request and Medical Information Release Form" and provide it to the patient.
  2. When the patient visits our institution, please inform them that they need to bring the "Infertility Treatment Request and Medical Information Release Form" and their health insurance card.

2. From the patient to our institution

  1. The patient should complete the "Patient Basic Information" section on the "Infertility Treatment Request and Medical Information Release Form" (it is acceptable if the medical institution fills it out).
  2. The patient should contact our institution to make an appointment (or you can contact us on behalf of the patient).
  3. On the appointment day, the patient should bring the "Infertility Treatment Request and Medical Information Release Form," their health insurance card, and any other necessary documents (if applicable).

[Important Note]
* Except for oocyte pick-up (OPU), most of the tests and procedures can be done on the same day of the visit.
* In vitro fertilization (IVF) requires a prior appointment.

3. From our institution to your institution

  1. After the patient's visit, we will provide you with a report regarding their examination and treatment.
  2. The content of the report may vary depending on the nature of the medical care and tests conducted.
  3. We are committed to ensuring appropriate communication with your institution and maintaining collaboration when necessary to continue providing comprehensive care.
