2. Please send us an email with your patient information

*We will send information about medical treatment (if any), payment information, etc. to the e-mail address you sent.
*Please note that you may not be able to receive e-mails from our clinic due to settings such as "Do not receive e-mails except from designated domains" or "Do not receive e-mails from PCs".

3. Submission of consent form and payment

We will send a consent form for the test and information about payment to the email address you sent the patient information.
Please fill out the consent form and return the PDF file or the image of the document you have filled out.
Also, please make the payment according to the guidance email.

4. Inspection Kit Shipment

After confirming the consent form and payment for the test, we will send the test kit to your registered address.
It will be delivered to the mailbox (depending on the application item, it will be delivered by Yamato Transport's courier service compact).
In consideration of privacy, the product name column of the delivery slip will be written as "Daily necessities (plastic container)".

5. Self-collection and mailing of specimens

Follow the instructions on the test kit to collect your own specimen.
When all the specimens are finished, put them in the special envelope provided and mail them in the mailbox.

6. Reservation for online medical treatment

Please call the "Oak Clinic Contact & Reservation Dial" 10 days after submitting your sample.
After confirming that the test results are out, we will make a reservation for online medical treatment.

7. Online medical treatment

At the time of your appointment, our clinic will contact you and the doctor will inform you of the results of the examination.
If you need a test result report, we will mail you a copy, so please let us know. [Paid (excluding tax): Postage fee ¥200 (¥220 including tax), ¥50 per report (¥55 including tax)]
If treatment is required, we will inform you about medications or prescriptions.

8. Shipment of test results report and medications

Please pay the cost of the test result report and the medications in the same way as in 3.Once payment has been confirmed, we will send the test result report and medicines to your registered address.

9. Re-examination（Only if necessary）

After taking the medicine, you will be retested to confirm treatment.
The same procedure as 1 to 8 will be performed only for the items that are positive.
