Notice Regarding the Discontinuation of the Frozen Storage Management Application Form

We sincerely appreciate your continued support of our clinic.

Until now, we have asked patients to complete the "Frozen Storage Management Application Form" at the time of payment for frozen embryos, etc. However, as of June 4, 2025, we have discontinued the use of this form.
Going forward, it will no longer be necessary to complete the application form.
